Information you choose to share

When you use our contact form, you may share your name, email address, practice or organization name, phone number and message.

When you complete the Free Readiness Check, you share your name, practice name, email address, phone number (optional), specialty and your yes / no / not sure answers. We never ask for Social Security numbers, license numbers or patient information through this website.

Questions typed into the Ask CareOne assistant are stored without your name so we can improve our answers.

Information collected automatically

When you click one of the official reference links in our Helpful Resources section, we record which link was clicked, the page it was clicked from and the time. This tells us which resources are most useful. It is not tied to your name or email.

How we use your information

We use what you share to respond to you, follow up on your readiness check, and send you a copy of your results when requested. We do not sell, rent or trade your information.

How we protect it

Submissions are stored in a secured system that only authorized CareOne administrators can access. We keep information only as long as needed to serve you, and you may ask us to delete it at any time.

Third-party websites

Links to outside sites such as CAQH ProView, the NPPES NPI Registry and the Federation of State Medical Boards open those organizations' own websites, which follow their own privacy policies.
